Developed by Women's Preventive Services Initiative, a federally supported collaborative program led by the American College of Obstetricians and Gynecologists, the Well-Woman Chart provides women information about preventive health care services recommended for their age group.  The Well-Woman Chart is updated each year and available in both interactive or print versions. Women can use it to prepare for their annual checkup appointment, knowing what health screenings they may need and gathering questions they might want to ask their doctor.

The Essential Person Program helps people who are blind, have a disability, or are 65 years of age or older stay in their homes by contributing to the cost of having someone live with them to provide essential care.  Call Vermont's Senior Helpline at (800) 642-5119 to get answers to your questions and help applying. An outreach worker can come to your home to help you fill out an application.

Medicaid for the Aged, Blind or Disabled (MABD) - (800) 250-8427

Medicaid for individuals who are applying based on being age 65 or older, blind, or disabled.

Prescription Assistance - (800) 250-8427

Prescription Assistance helps eligible Vermonters pay for prescription medicines. There are currently two programs: Healthy Vermonters whcih provides a discount on prescription medicines with no monthly premiums, and VPharm which helps pay for prescription medicines with affordable monthly premiums.

Vermont Health Connect is administered by the Department of Vermont Health Access, part of the State of Vermont's Agency of Human Services. Vermont Health Connect administers health care insurance plans in Vermont, including: Dr. Dynasaur; Medicaid for individuals who are not blind, disabled, or age 65 or older; and private health insurance plans, with and without tax credits or subsidies.

Provides oversight, consumer information, and investigates consumer complaints against insurance companies, (as well as securities, and banking industry, including credit unions, mortgage brokers, and other consumer financial products). Assists with consumer complaints about health insurance, managed care plans, or health care services received. DFR responds promptly and completely to consumers’ insurance and financial questions and complaints, assist consumers in resolving those complaints whenever possible, and helpsconsumers understand their options.
 

Operates the Healthcare External Appeals Process where consumers can request review of their health insurer’s decision to deny coverage for or payment of a service by an independent third-party not related to the insurer or consumer. An external appeal is available if the insurer denies coverage on the basis of medical necessity, or because it determines that the care is experimental or investigational. The external reviewer will either uphold or reverse the insurer’s decision, and the decision is binding on the insurer.

Outpatient Treatment: We offer substance abuse treatment for young parents in the community, including Medication-Assisted Treatment as part of Vermont’s Hub and Spoke treatment system.

Screening and Assessment: We perform comprehensive screening and assessment services for parental substance abuse in families where child welfare is a concern.

Family Support Services: These include parenting education; transitional housing; developmental screening for children; job training; case management; counseling for incarcerated mothers, their children and caregivers in the community; and supervised visitation for non-custodial parents.

The Family Assistance Centers (FAC’s) are designed to assist military families throughout all phases of deployment.  FAC’s are a “One Stop Shop” for service members and their families to obtain updated resources and supports available. This includes Emergency Financial Assistance, Support for Children/Youth & Adult Family Members, TRICARE Questions/Problem Resolution, Crisis Intervention and Assistance, Tax Filing Assistance, Problem Solving, Peer Connection, and Red Cross Notification Assistance.
